This form helps me get to know your horse’s history, routine, and current concerns before our first session. The more detail you can share, the better I can tailor bodywork and PEMF to what your horse actually needs.

Please complete one form per horse. If anything changes (new diagnosis, injury, saddle fit, shoeing changes, etc.), you can always send an updated note before your next session.
